SPORTS REQUIREMENTS

You must have the documents listed below completed and submitted to your coach or athletic director by the first week of the specific sport season.

  1. Physical Form ( Up to date for the school year ).  Every student athlete must have a yearly physical.  Free sports physicals are available at Denver Health School Base Clinic @ Lake.  They are at no cost to the student.
  2. Parent Permission Form – signed by parent or guardian.
  3. Emergency Contact Form – Completely filled out.
  4. $25 Pay to Play – Fee is required upon making the team and prior to participating in the first game for each sport.  Checks made payable to Lake Middle School.  NO REFUNDS.  The fees pay for the referees at home games, uniforms, equipment and a coach.

If the physical and parent permission/emergency contact form is not turned in, students may not be able to participate in tryouts or practices.

Stay tuned for the date and time of the parent meetings.  Look for information on sports in the weekly Lake dialer and “Mac Mail” for information on practice times and games.  All sports are coached by teachers and staff.  Please give them your utmost respect.  Practices, meets and games take place after school and occasionally on Saturday.

Eligibility

In order to be eligible to participate in athletic competitions,  students must have all passing grades ( No F’s ).  Eligibility will be turned in weekly for review.  Our dean will determine what steps are best for the student to become eligible on an individual basis.
